Domiciliary Care:
Domiciliary care is a range of services designed to support an individual living in their own residence. This package of care offers you assistance with activities of daily living such as food preparation, food serving, bathing, continence care, house chores or anything else that is needed to keep you comfortable in your home. In this domain, the needs tend to be multiple and once admitted as our Service User, a bespoke, Person-centred care plan will be drawn with you, next of kin and a representative of Lush Meadow Care.
This package of care allows you or your loved one to remain safe and comfortable in the environment they prefer the most – their home. Care workers who are fully trained, will regularly come to your home, as per your bespoke care plan to meet your needs. As all our Care staff also receive in-house training and induction, you’re sure to get the best from our services. With this in mind, you can be rest assured that your privacy, dignity, comfort and confidentiality, will be maintained throughout - in the comfort of your home.
Personal Care:
We understand that personal care can be sensitive to each individual and can differ greatly from one person to the other. You may find that you have an aspect of your life where you need assistance; such as dressing, bathing, medication or continence care. Lush Meadow Care Services, can make a huge difference to your quality of life. As all our Care staff will have undergone specific Caring training, before being recruited and Lush Meadow Care’s own in-house training and induction – you’re sure to get the best from our services. With this in mind, you can be rest assured that your privacy, dignity, comfort and confidentiality, will be maintained throughout - in the comfort of your home.
With your consent, we will liaise with your local pharmacy, as required, to put your medication in a Dorset pack; where accuracy in medication administration will be maintained by our Carers and your safety maintained. As new staff join our team, they will always shadow an experienced member of staff on their first visit. This is to promote a smooth transaction, continuity of care, an opportunity for both of you to meet each other in a controlled setting and for the new Carer to know your usual routine at your home together, with your plan of care.
Recovering from illness:
Most people prefer to stay at home while recouping following an illness or surgery. An illness can sometimes knock our confidence to live independently again. While you are going through this phase, Lush Meadow Care will be at your service; to assist you with your activities of daily living and any other needs you may have.
Upon receiving a referral to care for your needs, a Lush Meadow Care representative will come and assess you, even from hospital, where full access to your medical records will be sort to ensure a safe hospital discharge is made. This process also ensures that regardless of where the referral is coming from; your care needs are fully understood, the care plan is properly drawn; the care package is complete and comprehensive, to be cared for safely and comfortable at your home.
Once all this is done, you will then be admitted in our service, where, orientation to our service will be given to you and your next of kin. A Person-centred care plan will also be drawn, in collaboration with you, family and agencies involved in your care.
Speciality Care:
Please note that at Lush Meadow care, we have staff members who are able to meet some of your needs that are classified as specialty services. These are not applicable to every service user and may include;

As with all specialty services, there will be other Care agencies involved in your care; where liaison with those agencies will be paramount in drawing your care plan. Once, you have chosen and nominated, Lush Meadow Care Ltd to care for your needs; Informed consent and Implied Consent, will be automatically activated and applied, as we liaise with those agencies and in implementing the care plan, which will be drawn with your input.
It is, however, possible that there will be times when we are not able to admit every prospect Service User into our care, needing these specialists’ services due to high demand, as our health community services are overwhelmed.
Dementia Care:
Our team is here to support you and your family members when a dementia diagnosis has been made.
Family relationships can be profoundly impacted when a loved one is no longer able, to safely and independently meet their activities of daily living. A decline in cognitive capabilities, can also cause stress, anxiety and behavioural change in your loved ones. This is where Lush Meadow Care services come in; to develop a robust person-cantered plan of care in liaison with you, your family and other agencies involved in your care.
Being in a familiar environment, like your home, during this time maybe the best option and we commit ourselves to give you the best possible care; as remaining at your home, remains safe and comfortable for you.
Companion Care:
When people get older, they may feel isolated and lonely. This is mainly due to their loved ones passing away or living further away from family members. As the children grow, they may also relocate away from home due to work demands or their own family needs.
Through these difficult life changes, Lush Meadow Care Services can help close the gap. We can offer a companion service, for as long as you need during the day; where your needs and activities can be planned jointly with us. This service is depended wholly on your needs and the services offered may include, but not limited to; chatting, playing board games, listening to your favourite music etc.
Accessing Local Amenities:
As the care provided by Lush Meadow Care Ltd is strictly personalised; your needs will be well captured and catered for. If you are at all, in need of Lush Meadow Care, to access your local amenities on your behalf; we can do so, with any of the following tasks:

Weekly grocery shopping

Making General Practitioner (GP) and Hospital appointments

Arranging and booking registered transport to take you to and from your appointments

Requesting your regular prescription from your GP

Liaising with other agencies involved in your care, as we optimise your health outcomes and giving you feedback of every liaison
